A. After signing a confidentiality agreement we will send you a packet to describe the program. If you decide to participate please complete and sign the enclosed contract, enclose a copy of your current license, proof of insurance, list of office locations and return to ClearValue Hearing.
A. You will order, pay invoices, collect from patients as you do in your normal business except you will be assigned a ClearValue account number which will identify your discount pricing. This account number will be assigned by your inside sales representative at The Starkey Group. The patient membership number for their specific organization must also be included on all order forms.
A. ClearValue Hearing and The Starkey Group directly market to each organization for you. When a member activates their ClearValue plan you will be notified of those individuals in your area. It is your responsibility to schedule tests and make recommendations as you would any referral. Displaying and including the Farm Bureau logo in offices and marketing pieces will initiate additional referrals.
A. Each organization requires a Patient Satisfaction Survey to monitor the success and benefit of the program. Upon activation, the patient will receive the survey. Remind them that they must complete it and mail it to ClearValue Hearing to receive their batteries. We will send some additional Satisfaction Surveys to each provider so they can assist the member in completing it at the fit date.
A. After you’ve become a Preferred Provider you will receive a ClearValue price sheet which lists The Starkey Group suggested retail price and the discounted price that you can charge for each circuit. All models, CIC, ITC, ITE and BTE’s are priced the same and include directional microphones or telecoil and a two year warranty at no extra cost to you or the patient. All other options can be charged accordingly. Only maximum limits are listed to reflect differences in pricing and special promotions, however, your pricing to activated members must reflect a discount from your normal pricing policy.
A. Diagnostic evaluations are not a benefit of this program. If at any time during your exam you suspect a medically significant loss, inform the patient that before you can administer your free Hearing Aid test a diagnostic evaluation and or medical referral will need to be recommended, both of which are not covered by the ClearValue Benefits Plan.
A. Yes. The legal interpretation from the Attorney General's office is as follows: Persons who pay a membership fee or are employed by a company that has a contractual relationship with a providers group can receive free services and special pricing because of that member or employee relationship
A. Yes. While not required, it is highly recommended that you include organizational logos and contracted manufacturer’s names in your normal marketing pieces. ClearValue Hearing and The Starkey Group specifically market each organization through mailings and advertisements in publications to support this program.
A. The Starkey Group is the exclusive instrument manufacturer for ClearValue Hearing, and activated members must be fit with The Starkey Group products and be billed no more than the maximum allowable price. Free Hearing Aid testing, periodic clean and checks are also included benefits. Please refer to the included Providers Responsibilities section for specific items.
A. Both parties have the right to terminate this contract at anytime, with a 30 day written notice signed by the appropriate individual.
